Health care within
the Veterans Health Administration (VA Medical Centers) System is
available to any honorably discharged veteran. Access to health
care requires the applicant to be enrolled in the VA Health Care
System. Enrollment requires the applicant to complete:
"VA FORM 10-10 EZ APPLICATION FOR HEALTH BENEFITS"
and submit it to the nearest VA Medical Center with proof of service
( report of separation, or dd214 if service was after 1 January
1950).

If you set foot on land in Vietnam, or if you were authorized to wear either The Vietnam Service Medal or Armed Forces Expeditionary Medal ( for service in Vietnam )
between 1-9-1962 and 5-7-75, check "YES"
