Veteran's Health Care Enrollment

Health care within the Veterans Health Administration (VA Medical Centers) System is available to any honorably discharged veteran. Access to health care requires the applicant to be enrolled in the VA Health Care System. Enrollment requires the applicant to complete: "VA FORM 10-10 EZ APPLICATION FOR HEALTH BENEFITS" and submit it to the nearest VA Medical Center with proof of service ( report of separation, or dd214 if service was after 1 January 1950).

When completing VA FORM 10-10EZ (Application for Health Benefits) please note the following: On page 2, SECTION VI Military Service Information. In Item 2. CHECK YES OR NO . Question "G" : Were you exposed to Agent Orange while serving in Vietnam ? If you set foot on land in Vietnam, or if you were authorized to wear either The Vietnam Service Medal or Armed Forces Expeditionary Medal ( for service in Vietnam ) between 1-9-1962 and 5-7-75, check "YES" TO PRINT AN ENROLLMENT FORM: 1. Click on "Printable Form" 2. Follow the instructions in "View or Print Form 10-10EZ" ( you'll need Adobe Acrobat Reader 5) You can also obtain an enrollment form at the:       County Veterans Service Office.       Sheboygan County Courthouse       Room B-4       615 N 6th St..
