	This application must be completed, signed and dated by applicant Select the desired print format to be used Application may be filled out on your PC using either the Word or Acrobat Adobe® formats      Or, the application may be printed and filled out manually Please type, or print neatly in ink, the required information on the application form Enclose the Permit Application Fee Submit the application to the Sheboygan County Highway Department, at the address shown on the application form
