	Hours of Operation: Hours:  Monday thru Friday   8:00 AM - 5:00 PM Closed - Saturdays and Sundays

UNIT GOALS: To provide comprehensive case management in order to: Facilitate increased consumer participation in program development, his/her treatment planning and goal determination. Maximize independent living and problem-solving skills. Coordinate and Facilitate initial, ongoing & aftercare services for clients Reduce need for hospitalization/rehospitalization.

UNIT SERVICES: Assessment Case Management - comprehensive service coordination* Activities of Daily Living Training - (in-house/community) Counseling Crisis Intervention Psychiatric Services Coordination/consultation with other providers, institutions and courts

ELIGIBILITY REQUIREMENT: Adult resident of Sheboygan County (18 years of age and older) Diagnosed with multiple disabilities (combinations of MI, DD, OBS, AODA) Ineligible for CSP or DD services Need for assistance in meeting goals as described under UNIT GOALS

FEES: Persons receiving services are responsible for the cost of services. Payments may be through personal insurance, Medicaid or personal means. A reduced payment may be determined based on income, family size and related circumstances per Wisconsin Uniform Fee System available upon request.
