	Hours of Operation: Hours:  Monday thru Friday   8:00 AM - 5:00 PM Closed - Saturdays and Sundays

UNIT GOALS: The unit goal is to provide services that will allow individuals to maximize their independence in the most facilitative setting.

UNIT SERVICES: Inpatient Services Determine need for inpatient or extended care services at Memorial Medical Center, state institutions (Winnebago or Mendota Mental Health Institutes) or Institution for Mental Disease (Sheboygan County Comprehensive Health Center); authorize payment when needed and appropriate, provide service coordination and discharge planning as needed and arrange for community services when appropriate. Residential Services Provides consultation and liaison services to contracted facilities to ensure that optimum services are being provided.

ELIGIBILITY REQUITEMENTS: Any Sheboygan County resident who through assessment and evaluation meets the specific program criteria.
