UNIT GOALS:
To provide an accessible, comprehensive, effective system of services in the community for Sheboygan County individuals with severe and persistent mental illness in order to:
- Facilitate increased consumer participation in program development, his/her treatment planning and goal determination.
- Increase community living proficiency through improved interpersonal, vocational and daily living skills, and use of community resources.
- Decrease measured psychiatric symptomatology.
- Reduce hospitalizations, increase community living options

UNIT SERVICES:
- Assessment, Treatment Plan Development, and Plan Monitoring
- Counseling/Therapy - Individual, Group, Family
- Case Management - Comprehensive Service Coordination
- Outreach
- Psychiatric Services
- Medication - education, monitoring, supervision and administration
- Interpersonal, vocational and daily living skill training
- Crisis Intervention
- Coordination/consultation with other providers, institutions and courts

ELIGIBILITY REQUIREMENT:
- Resident of Sheboygan County (18 years of age and older).
- Diagnosed as having a major mental illness.
- Impairment in one or more of the following areas of functioning:
- Vocational, education, homemaker
- Social, interpersonal, community
- Self-care or independent living

FEES:
Persons receiving services are responsible
for the cost of services. Payments may be through medical assistance,
personal insurance, Medicaid or personal means. A reduced payment
may be determined based on income, family size and related circumstances
per Wisconsin Uniform Fee System available upon request.
