	Hours of Operation: Hours:  Monday thru Friday   8:00 AM - 5:00 PM Closed - Saturdays and Sundays

UNIT GOALS: Assist consumers in their effort to maximize their independence through a consumer driven, recover oriented, strength and outcome based program: Empower consumers to take an active role in their treatment and recovery. By increasing independent living and problem solving skills. Facilitate/coordinate initial, ongoing, and aftercare services for consumers To develop/increase natural supports in the community so that consumers will be less reliant on county systems.

UNIT SERVICES: Services offered through CCS are individually determined by the Assessment and Recovery Plan. Assessment Service Facilitation – Comprehensive Community Service coordination (i.e.: connect to employment-related skills training, medication management, etc.) Recovery Education Counseling Psychiatric Services Crisis Intervention Ongoing Coordination Coordinate/Consult

ELIGIBILITY REQUIREMENT: Available to individuals who are determined by the State-approved Functional Screen. Who can benefit from/need psychosocial rehabilitative services. It is a program of choice and individuals choose to enter CCS if the above criteria is met.

FEES: Persons receiving services are responsible for the cost of services. Payments are through Medicaid. A reduced payment may be determined bsed on income, the family’s circumstances per Wisconsin Uniform Fee System available upon request.
