Services Offered at the Sheboygan County Health
Care Center

Rocky Knoll Health Care Center offers a broad range of
short and long-term therapy services. Each service has been designed
to maximize an individual's ability to function, enabling them to
reach their highest level of independence as quickly as possible.
Program features include a full-time in house therapy team, community
re-entry assessment, weekly psychiatrist review, and development
of care plans from active physicians and personalized treatment.
A consultant dentist is available for all residents if they desire. A dental exam room is
located at each facility where an initial exam will be performed and subsequent dental care
will be provided as necessary.
In addition to dental care, optometry, podiatry and diagnostic services
are also available. The consultant podiatrist is in the building
monthly to meet residents foot care needs. Any diagnostic service
is provided only on the request of the attending physician, and
all reports are included in the clinical record.
Activities for better living include bingo, cooking groups, music groups, snack bar, social
hours, outings and field trips, music therapy, pet therapy, art therapy, resident council
and family and resident support groups.
A registered dietician oversees the selection and preparation of meals to assure a varied
and well-balanced diet. Any individual with special dietary needs will be given careful
attention.
The professional nursing staff at each facility is skilled at providing
specialized care to the elderly and mentally ill. Personal care plans are developed to accommodate each
resident's needs, emphasizing each individual is unique and deserves
special attention. Physicians, nurses, therapists, social workers,
activity therapists and the dietician develop the care plans. The
care plan will aid in daily living activities as well as management
of medical problems.

Pastoral Services at the
Sheboygan County Health Care Center

The Chaplain, Rev. Tom Fleischmann, and Pastoral Services staff are committed to supporting the faith that has sustained the lives of our residents. It is the goal of the program to help residents spiritually as they face physical and emotional challenges in life.
The regular worship services are a vital part of the resident's life. The Chaplain and other local clergy hold them through out the month. Lutheran, Catholic and Protestant services are held, along with special services for Christmas, Good Friday, Easter and Thanksgiving. Other services are planned occasionally for Veterans and special events.
Memorial services are one way to say how important the people who
come to live at the Health Care Center are, and how, on many occasions,
these residents become very close. All residents, family members
and staff are invited to attend. Rocky Knoll has
the services monthly. The service times are posted for the residents
and invitations are sent to the families.
There is a pastoral service room located at each facility for counseling and gathering the family together in a time of crisis. This private setting is for the family and their pastor to be together at a difficult time. Tracts, pamphlets and Bibles are there for family and friends to use.
The Pastoral Services Volunteer staff is an essential part of providing spiritual care for the residents. They lead Bible Studies, visit and read to residents one on one, provide music for worship and escort the residents to and from the Chapel services.
